This is the official enrolment form. It’s available in English only.

Information on the Get Ready to Vote form is collected by the Electoral Commission for electoral purposes. You, or the organisation you represent, must not retain personal information provided on the form, or use it for any other purpose.

Completed forms should be returned to the Electoral Commission as soon as possible so we can process and confirm the enrolment.

People can return their forms directly to us by post, email or upload to our website. The details are on the form.

You and your organisation can also return completed forms to us. If you scan and upload or email them, please make sure the original forms are securely disposed of.
